When i used to set the cnc machines I Know its old hat now But i used a 5mm setting peice not my best slips . using electronic hand Wheel wind the quill down carefully take a light touch . go to controller Datum set z + 5 . As martin says
Always s think in terms which direction the cutter is moving z- x+ y- etc . drifting away a bit wire eroders you can program the bottom different to the top say for backing of dies
  I would have thought the knee up on a milling machine would enable the slot drill / cutter  to take another 
peck/cut  therefor Z minus .  I have my flack jacket on ready for replies

As Rick says soft centreHere is a live centre . When in chuck as it is soft can be turned true using top slide set at 30 degrees

Why not screw cut . I cut 5/32 whit over about 7"travelling steady" on my S7 Mk one Using a traveling steady

cant help with spindle . But I notice in No. 2 photo . there is a large gap between the head and the stop on the pillar . elf and safety . slide it back under and lock in place . if you swung the head round at the moment the head would drop and my do some damage or injury . Please accept this in the spirit of this forum

Looking at the photo is the the right side cut with the ram is out . indicating that there it play in the ram when extended . May be judder in the ram What shape of tool are you using
