Repairing a D hole

Has anyone got any tips for repairing a stripped D-hole? The damage was caused when the output gear jammed and the shaft continued to rotate, chewing out the flat on the D-hole.
 
I have ideas about filing a flat in the hole and making a packing piece to fill the gap, but the filing will not be a precision operation (by me).
 
Are repair inserts available? This would allow me to drill out the hole and then glue an insert in place.
 
Any help much appreciated.

Fixed it. I filed a slot, the width of the flat, where the flat on the D used to be. This was 2mm deep ie below where the D used to be. Then I made a key 2mm thick and 0.3" wide (width of flat). With a bit of dressing to give some clearance, the key slid in and did the job.
